This authorizes Applied Business Solutions to send credit entries electronically or by any other commercially accepted method to your account and the financial initiation holding the account to post all such entries.

Voluntary Payroll Deduction Authorization
This document grants an employer authorization by the employee voluntarily to deduct a specified amount from the employee’s payroll per pay period.
